I Need to have my iphone 5 battery changed and they said I need to back my phone up. Do I need to back up all my apps and if I don't will I loose those apps that I didn't ?

You should have them in the iTunes library on your computer anyway, but if you're just backing up to iCloud, the apps will re-download when you restore from the iCloud backup if that's even necessary.
You probably won't need to restore from backup afterward (I didn't when my battery was replaced), but you NEED to make sure you have a backup before you take it in for battery replacement.
It's always a good idea to backup your data regularly.
