OS X 10.8.5 with Server I've installed the server 2.2.5 update but AppStore still shows it as an available update. Restarted the system, same result. Does this mean that the update hasn't installed correctly or is it just an error in AppStore?

What I have found is that if you have a copy of the Server App on another drive someplace as a backup this will register for the App store to be upgraded if that folder is within the folders that Spotlight monitors.
Once I deleted my backup folder I no longer had this appear. If you do not want to delete it then remove it from Spotlight.
Search "This Mac" in the Finder for outdated copies, and delete any you find.
Did you actually start the Server app after installing it? I saw the same thing and when i actually started the Server app it indicated that it needed to go through a configuration process to complete the installation.
This has worked for me a couple of times now. Not sure why it keeps coming back but I am sure it is my fault since I maintain multiple backups and keep changing machines and drives.
