How do I disconnect an old iPhone from sharing apps, files, FaceTime, emails through my MacBook Pro when I don't have the old phone?

I had an iPhone 4S for work that was replaced by an iPhone 6. The old iPhone was synched and shared files, apps, FaceTime from my additional personal mobile number and my MacBook Pro and after handing back realised that it was still synched to my other devices. How do I disable the old iPhone, without having it available?
