The reason you are seeing that it is being used by something else is because it probably has to do with an Apple TV, Bonjour, AirPlay, Home Sharing, Printer Discovery or Back to My Mac. Take a look at the first two articles as they talk about what those ports are used for by Apple and the last article will walk you through on what to do if you are having issues with port mapping.

Yes, I had already read all of those before posting my question. Back To My Mac is already disabled on all of my devices, and is only relevant to port 4500. The fact that it has to do with Home Sharing, AirPlay, Bonjour, etc is the reason I need to open this UDP port.

That is, what is the make & model of the device that it is connected to for Internet access? Based on your initial comments it sounds like this device is not a simple modem but a combination modem & router or gateway instead.

Comcast Internet > Motorola Cable Modem > Airport Extreme (ac) > OS X Server 4.
Nothing unusual, and everything else works exactly as it should in regard to port forwarding, however UDP port 5353 appears to be the only port I can't forward on the Airport Extreme.
