If you are not seeing Personal Hotspot then take a look at the article below and start at step 7. Start with resetting the network settings on your iPad and if that does not work then you will need to restore it. Let me know if this works for you.

don't worry this problem can be fix
trust me
first take backup if you trust me
guys first of all you need to do (reset all content) you device
then reactivate you device on WiFi not with iTunes
and careful it will prompt update carrier settings don't update that
don't update your carrier settings every time it prompt
that's it when you connect your device to iTunes its will ask again on iTunes just cancel it
after that if you don't know where is personal hotspot you have to configure your APN.
goto iOS>settings >cellular>cellular data>scroll down you will see personal hotspot APN type in APN what ever you want and go back to home screen
open settings>cellular>cellular data>and you will see there is personal hotspot
just remember don't update Carrier settings
