Format the HD as Mac extend journeled and select 1 partition>advanced> GUID (GPT) . All data will be erased on the drive.
After formatting, reinstall the OS X from there. Upgrade if need be.

In that case you will need to reformat from the installer DVD that shipped with your Mac initially. You can order from Apple or take it in for assistance
