Further research shows it may be related to an app called "Private Internet Access" somewhere on your computer.
Check to see if you have it or not. See this thread and follow Linc Davis's advice.

The System Prefs/Network suggested repair has shown some promise...it changed to a different window from Telstra (Wi Fi supplier) and, after some fiddling, it seems to have disappeared but Mail is now not working. That should not be as hard to fix. Hopefully when Mail is working again, the window won't come back!
Have also tried to find an app called 'Private Internet Access' without success. Anyway, some progress.

Regarding that thread, I would not recommend following the advice near the bottom about messing around with Terminal commands. As Ralph says, it could be in Utilities. Also, it may not be an app but a plugin. Search in Spotlight, and if that fails, there is a very handy app called Easy Find that you could try. https://itunes.apple.com/us/app/easyfind/id411673888?mt=12
If you find that there is no such app or plugin on your Mac, it could be a Network preferences problem. You may find this OSXDaily article helpful. http://osxdaily.com/2014/10/25/fix-wi-fi-problems-os-x-yosemite/
