(15-inch, Mid 2012)5 5 4 - - - Was the Bootcamp info.plist modified on this Mac? How was the OS installed when the original disk drive failed?
Can you click on System Report -> SATA/SATA Express and PCI to see if your internal drive shows up? Do you have more than one physical drive in this machine?

There is a 15" non-Retina and a Retina model for 2012. Do you have the non-Retina model?
Can you post the output of the following commands?
diskutil list
diskutil cs list
sudo gpt -vv -r show /dev/disk0
