Music all over computer - need to consolidate

I am using Yosemite on my MacBook Pro and iTunes 12.0.1.26, I have music all over my mac and want to consolidate it in one place. As you can see below, one song (that I am using as an example) has all of these file paths -

MacIntosh HD - Music - Itunes Music - Artist - Album - Song

MacIntosh HD - itunes - itunesMusic - Artist - Album - Song (have this one twice)

MacIntosh HD - Artist -Album - Song

MacIntosh HD - Music - itunes - iTunesMusic- Music - Artist - Album - Song

MacIntosh HD - music transferred - Music - Artist- Album -Song

my name - new tunes612 - iTunes Media - Music - Artist - Album - Song (in 2012 my hard drive crashed on another Mac - got files in one place to move over)

my name - iTunes Media -Music - Artist - Album - Song

I am not sure why I have this problem other than the folder of new tunes612 which I described above.

If someone could tell me how to consolidate my music in the proper place (whichever that is) I would be grateful.

Thank you, smaxie29
