Having gone through these forums searching for an answer, I can see this is a well documented problem that needs to be addressed by Apple.
Here's to hoping they will look into it and provide a fix soon!

IPad Mini R
connected to my HU by lightning.
Randomly drops volume, like you say, it's like when you get a notification.
Only way iv found to clear the issue is by quitting all music apps (double tap home button and slide up) and opening again. then the volume goes back to normal. Very annoying!
