Why is my phone displaying the wrong time

My phone is consistently showing the wrong time. It shows the correct time after I restart it, but soon shows another incorrect time. Usually 15-60 minutes behind current time and sometimes not changing. I.E it read 12:47 for 10 minutes or so.

I did a full restore, but it keeps doing it...
