So, I want to send a simple game to my cousin that broker her shoulder. From what I read it use to be very simple but there seems to be no "Gift" button anymore. So is there a way I can buy and send her an app as a present now? (other than giving her money and having her buy it).

From your post I see that you would like to gift an app to your cousin. The gift option still exists, as the instructions are outlined below.
Gift an item on an iOS device
- Find the item in the App store or iTunes store that you wish to gift.
- Tap the Share icon in the upper right.
- Tap the Gift icon.
- Sign in to the iTunes Store with your Apple ID.
- Type the email address for the person you want to send the iTunes Gift to.
- If you want to, add a message.
- Pick the day you want to send the gift.
- Select a theme for your gift.
- Tap Buy, then tap Buy to confirm.

Well, this sort of helped. I wanted to buy her a game that I owned because I knew she would like it. But the "Gift" icon never shows up. So I tried it on some random game and there it was big as life a "Gift" icon. So apparently you cannot gift a game to someone that you have already purchased in the past. That's too bad.
