If I understand correctly you updated both your 6+ and your wifes 4s and the call quality dropped sharply thereafter. I would use these steps specifically on both phones from the following article:
If you can't hear a person on a call or voicemail or if the sound quality is poor on iPhone
- Restart your iPhone and test again.
- If there's still no sound or poor sound quality, update to the latest version of iOS.

If you've tried these steps and there's still no sound or poor sound quality from your iPhone, contact Apple Support.
Seems a bit simple I know, but the rest of the troubleshooting doesn't seem to apply if this happened after a software update, although they are worth a look over and double check to make sure.
I would also even go as far as backing up and restoring the phone using iTunes before contacting support:
Back up and restore your iPhone, iPad, or iPod touch using iCloud or iTunes

Thanks for your input. The problem got so bad yesterday with missed calls, even though it's in your hand, bad reception and deleted SMS messages that I've gone back to 8.1.2, and to date the problem seems to have gone away on her 4s.
Jury still out on the 6+ which has retained the 8.1.3.
