Please unmount the internal disk, before you run Verify/Repair via Disk Utility.

Can you boot from USB and post the output of the following Terminal commands? For "sudo" commands, enter your password, and it will not be echoed back to you.

    diskutil list

    diskutil cs list

sudo gpt -vv -r show /dev/disk0

    sudo fdisk /dev/disk0

    1. If you notice, the diskutil list shows 30GB missing.

    2. You have a 30GB gap between GPT3 and GPT4.

    3. Your MBR (Fdisk output) is incorrect, unless Windows was installed using EFI boot.

    4. If you boot from an external USB, can you post diskutil list once you have it up and running. Please see OS X: Installing OS X on an external volume - Apple Support on how to build an external boot disk for OSX. If you have a second Mac, you can also use Target Disk Mode - Share files between two computers with target disk mode - Apple Support.

    Thank you -

I booted from external USB. The diskutil list output is from OS x booting from the USB, which shows the following:

    Saeedehs-MacBook:~ saeedehmirbagheri$ diskutil list

    /dev/disk0

       #:                       TYPE NAME                    SIZE       IDENTIFIER

       3:                 Apple_Boot Recovery HD             650.0 MB   disk0s3

       4:       Microsoft Basic Data                         50.0 GB    disk0s4

    /dev/disk1

       #:                       TYPE NAME                    SIZE       IDENTIFIER

       0:      GUID_partition_scheme                        *15.6 GB    disk1

       1:                        EFI EFI                     209.7 MB   disk1s1

       2:                  Apple_HFS BOOT                    14.8 GB    disk1s2

       3:                 Apple_Boot Recovery HD             650.0 MB   disk1s3

    Saeedehs-MacBook:~ saeedehmirbagheri$ diskutil cs list

No CoreStorage logical volume groups found

I am not able to use target disk mode because I don't have firewire/thunderbolt ports.

You need a space between "Macintosh" and HD and the back-slash is to allow the space. You can also try

    fuser -c /Volumes/"Macintosh HD"
