Apple Id problem

I want to access Pacific Blue Cross and submit some forms. My computer wants me to allow my Apple Id to be used. I click Deny but it won't let me leave the page. In fact it freezes the page. Virus?

MacBook Pro with Retina display, OS X Mavericks (10.9.5)

Do you use iCloud Keychain in Safari? And have you stored a password for your Pacific Blue Cross account in iCloud Keychain?
