I did something and one of my app icons disappeared from my phone. The app is still there because when I double click on the home button and it opens up what apps are open, that app is there. It's just the icon that is missing. How do I get it back?

Did you drag it over another icon to create an App folder? or move it to another screen?
If you can't fix this try: Settings > General > Reset > Reset Home Screen Layout.
It was originally in a folder. Most of my apps are in folders on my phone. I looked in all the other folders and I can't find it. The app is still there, just the icon is missing.
1. Restart your phone and see if the icon should come back.
2. Check Settings > General > Restrictions, make sure Installing Apps is allowed.
If it is, try Settings > General > Reset > Reset Home Screen Layout.
I lost an app icon. The app is still there but the icon is gone.
