You will need to remember or find the id that you using in Settings > iCloud to be able to reactivate the phone. If you can't remember your account then do you have any of its downloads in your computer's iTunes library : Recovering a forgotten iTunes Store account name - Apple Support

If not then try finding it via http://appleid.apple.com : How to find your Apple ID - Apple Support

If you still can't find it, and you have proof of purchase of the phone (e.g. its receipt with its serial number on it) then try going to an Apple store and see if they can help (probably best to make a Genius Bar appointment), or if you don't have one nearby try contacting iTunes Support and see if they can help : http://www.apple.com/support/itunes/contact/

When you've found the id you can reset its password via http://iforgot.apple.com

(I've asked for your post to be moved to the Using iPhone forum, you've posted in the Using iPad forum.)
