Hi, My iphone doesn't work, its a Iphone 4 and doesn't charge properly. When i plug it in to charge it the logo appears for a couple second then turns off and does it again. And when it does work it on works for 1 min and 15 sec then it shuts off

Try resetting, restoring from backup, and restoring as new in that order. See if the problem goes away after each step. If that does not help, then the phone might have a hardware problem. Take it in to an Apple Store or authorized service provider for evaluation.

You get the PUK from your carrier. It might be available if you log on to their online account management site (if they have one). Otherwise you will have to call them.
