MacBook Air screen freezes

MacBook Air screen freezes

Ive had my MacBook Air for 19 months and recently the screen will shake and horizontal lines appear, the MacBook is then unresponsive and restarts. When I try and turn it back on again it seems fine for less than a minute and then starts again. Does anyone know what I can do to fix this?
