After the upgrade to 8.2, when dictating text through Safari into an open field, once you press "Done" to end Siri dictation, all text entered is deleted.
We have tested this multiple times with both 8.2 based iPhones (5S, 6 and 6+). All 8.1 based units work fine. All 8.2 units fail.
How does one go about reporting such an issues to Apple?

I am having the same problem on my iPhone 5, first appearing after the upgrade to 8.2. Any dictation into an open field in Safari is erased as soon as "done" is entered.
I have this problem, too. Siri deletes whatever I say in notes, email, web searches, etc.
This is a big issue. I use Siri to fill web form text at work. Since upgrading to iOS 8.2, all text filled by Siri is lost when entering Done or when Siri has timed out. The only workaround I have found is to hold my finger over the text rather then pressing Done.

I am having the same issue in IOS 8.2. The dictation feature is useless because it erases everything i have written, or certain characters. Now i cant use it in MS Word. This is a big problem for ppl that heavily relies on it for writing messages, articles, etc. Please we need a soon fix Apple!!
Apple responded to my complaint. They know it is a bug, they suggest holding your finger on the text to save it rather than press "Done" (and lose it), or press the ".?123" to go to "undo" that will bring the text back if Siri has timed out or erases your text.
I have a related issue. About 2 iOS updates ago Siri worked well for advancing the text to the next sentence and capitalizing the first letter after I dictate "period" or any other terminator at the end of the sentence. Now Siri usually places no space after the last sentence and starts the new sentence in lower case.
