Tethering Not Stable After Upgrading iPhone 5s to iOS 8.2

After upgrading my iPhone 5s to iOS 8.2, tethering becomes unstable. The device (MacBook Pro running OS X 10.10.x with the latest security patch or iPad running iOS 8.1.3) would connect but it would loose the connection after a while (no specific time period). When this happens, the iPhone still thinks the device is still connected (according to the Personal Hotspot state at the top).


This scenario worked perfectly before the iPhone was updated.
