S.M.A.R.T. Status failing

So my macbook pro non-retina 13'' mid 2012 has been stuttery and very laggy and I was going to completely wipe the computer and reinstall OSX, but decided to check the disk utility first and it's telling me

"This drive has a hardware problem that can't be repaired" and "S.M.A.R.T. Status failing"

I already backed everything up, but I was wondering if applecare would cover this problem? I bought my macbook in Nov. of 2014 so it's still less than a year and I'm considering buying it if it covers it.

If so, is this the correct one to purchase for my mbp?

If the S.M.A.R.T status is failing, then your hard drive may be about to fail. If DU is not able to repair it, you could try Disk Warrior.

If the machine is a 2012 and you bought it in 2014 - did you buy it as a refurbished unit from the Apple online store? If that is the case, then you are still covered by the one year warranty from Apple.

Or where did you get it? If you purchased it from a reseller, you would need to prove your purchase to Apple and if it was new, you are covered. If you purchased it used or refurbished by a reseller, then you cannot purchase Applecare.

As for the Applecare, I have no idea if that is the correct one (it does appear to be); my favorite place to get it is here:
