Was it set on the iPad when you last backed up ? If not then you can restore to that backup and it should be removed. If it was on the iPad when you last backed up then the only way to remove it is to reset the iPad back to factory defaults and you can then re-sync your content back to the iPad - you won't be able to restore to the backup as that will keep the code in place.

If you do have to reset to factory defaults then first copy any purchases over to your computer's iTunes library via File > Devices > Transfer Purchases, and copy any important documents, photos etc off.

You may need to put the iPad into recovery mode to be able to reset it back to factory defaults : http://support.apple.com/en-gb/HT201263

You can copy iTunes purchases over to your computer's iTunes library via the File > Devices > Transfer Purchases menu option

To copy off photos taken with the iPad (which is not done via iTunes) : Import photos and videos from your iPhone, iPad, or iPod touch to your Mac or Windows PC - Apple Support

How you copy documents from your apps depends upon what the app supports e.g. some allow you to copy content to/from them via your wifi network, some use emailing, and some use the file sharing section at the bottom of the device's Apps tab on your computer's iTunes : About File Sharing on iPhone, iPad, and iPod touch - Apple Support

To copy photos off, and to copy documents via file sharing, see the two links in one of my above replies.

To reset to factory defaults, with Settings > iCloud > Find My iPad enabled you can log into the account on http://www.icloud.com and send a remote wipe message to the iPad (the iPad will need to be online), or use recovery mode and reset it via your computer's iTunes : http://support.apple.com/en-gb/HT201263

I have forgotten restriction passcode store.
