If this happens on every call, it could be a hardware problem. If it happens on only some calls, the carrier network could be the source of the problem.

Does this happen on every call?

It does sound like a hardware problem, but take a look at this article -> https://support.apple.com/en-us/HT201559

Resetting your phone might help.
