iPhone 5S, iOS 8.3, passcode protected: even if the medical ID option to show on lock screen is enabled, the link does not show... Is there a trick to enable this that I am missing or is this feature not working yet?

From the lock screen you need to "Slide to Unlock" to get to the "Touch ID or Enter Passcode" screen (the one with the passcode keypad), then tap Emergency in lower left to get to the "Emergency Call" screen and then you should see Medical ID in the lower left.

Well, that is my problem, it does not show in the emergency screen. Yes the iPhone passcode is in use and yes, I have created a medical ID...
It sounds like you've done everything that should be needed. It is so well hidden that I thought you just may not have known where to find it after it was turned on. It is working on my 5s with 8.3
