Accidentally Blocking Callers on iPhone... repeatedly?

I occasionally block spam numbers that call me. Twice now, I've somehow managed to block my husband and my CFO. I talk to my husband daily but rarely talk to my CFO.

As far as I know, you have to go into the contact profile and click block this caller and then click block contact again. It doesn't seem like that could all happen by accident.

Has anyone else had this happen? I'm trying to avoid this from accidentally happening again...
