MY HEALTH DATA is Gone!
After erasing my phone, rebooting, and restoring from my backup with iTunes everything came back EXCEPT MY HEALTH DATA!
A soft reset does not help.

How can I get my data back?

Except backuping with iTunes, do you have backuped your iPhone with iCloud? If so, you may try to restore your health data from your iCloud backup.
