iTunes stops working after it asks for payment verification

I re-installed iTunes v11 on my Win 7 PC (after the upgrade to v12 was totally unsuccessful). When I clicked on the desktop icon, iTunes started up and then asked for my Apple ID password. It then stated that verification would be required if I wanted to purchase from the iTunes Store. I clicked OK and then iTunes stopped running. I tried this unsuccessfully several times. Any advice/help would be appreciated.

iPhone 6, Windows 7
