Unable to reuse company iphone.

Unable to reuse company iphone. The user of the iphone no longer works for the company and now I cannot re-use the phone because it needs to be activated. I cannot continue without filling in the username/password on the "activate iphone" screen. The phone has been reset by holding down the two buttons while starting but it still starts up with the screen. How can I re-use this iphone 5?
