How to get iCloud Calendar to show up in Outlook 2015

I am using Outlook 2015 on a MacBook Pro.

The calendars for my Exchange and Gmail accounts work perfectly, but I can't see my iCloud calendar in Outlook. It works perfectly on my iPhone.

Is there some kind of "iCloud add-in for Outlook" or setting that is incorrect?
