Facetime has disappeared from my 4s. How do I fix it? There is nothing about Facetime showing in my settings or contacts. The only place it exists is grayed out when I'm on a phone call.

Probably because it is banned in your country.
Or restricted in Settings>General>Restrictions.

You need to check restrictions on your device. That is the only reason that FaceTime would be greyed out.
Check: Settings - General - Restrictions - FaceTime = ??? Camera = ???
