FaceTime icon disappeared on iPhone 6 plus. How do I find it or download?

Face Time icon has disappeared from my iphone 6 plus. Can anyone please help me on where I can find it or Install again? I am assuming with the updates it somehow got deleted. Thanks.

Check: Settings - General - Restrictions - FaceTime = ??? Camera = ???
