duplicating an app on both iPhone 4 and 5?

I want to retain my app for keeping passwords secure on BOTH my iPhone 4 and iPhone 5. I have copied the app from the iPhone 4 onto the iPhone 5 but the data has NOT been copied across. How can I keep both the app and the data synchronised? I have different apps on each phone as they are used in different countries.
