why can't is see the menu bar at the top?

I don't see a menu at the top of my ITunes (store, etc...).

Press Alt to reveal it temporarily, or Ctrl+B to show/hide it.

Did not work. I can't see the menu bar in any of my applications, i.e. in safari file, edit etc.

That particular fix was required for some builds of iTunes 12 for Windows, though not the current one which no longer hides the menu bar. Which version of which operating system are you actually running?
