All my playlists erase from my phone when I sync! How can I transfer my playlists from my iPhone to iTunes on iOS 8.3?

I have been struggling with this all day and have had to completely restore my phone multiple times trying to figure this out! I have created multiple playlists on my phone, but they do not show up on iTunes at all when I connect, and if I try to sync, they all erase. How can I get my lists to transfer from my phone onto iTunes? (I have an iPhone 6 and 5s, both are running iOS 8.3).
