    Are you too close to the object? Have sufficient light? The camera lense and sensor nice and clean?

     

    Reset your device by holding down the Wake/Sleep and Home buttons until the Apple logo.

    If it's a mechanical issue, only an Apple a Genius at your local Apple Store will be able to diagnose it properly. Or use your warranty/ AppleCare to have it serviced.
