How do you do a link within a document

I write publish books. How do you do you put in a link from one part of a document to another: A chapter listed in the Table of contents to the chapter.

I have v. 5.5.3 I knew how to do that in an earlier version but can't figure out the process in this version. If you can with this version. The help was no help in this case.
