how do you stop voice echoing during calls?

Every time I make a call or someone calls me, the person on the other end always hears their voice echo. It is really annoying for them. I don't hear any different but whoever I speak to they make the same complaint. It has been doing this ever since I first got it a year and a half ago. Help
