Q: iTunes crashes Yosemite 10.10.3 to black screen

Yosemite 10.10.3 crashes to black screen then reboots automatically when I open iTunes. This also happens when trying to open Google Chrome.

This all started last week on Friday, Apr 24th. I had been having problems updating the Pro Video 2.0.1 pack from Apple's Software Update. Then I noticed Chrome had stopped loading pages at all and after a while all open apps would freeze until crash to black screen. The only way to reboot was to unplug. This still happens with Google Chrome. I uninstalled using App Cleaner and trashed all Cache files associated with Chrome. After re-installing Chrome the same problem exists in the new install.

I then booted into Recovery Mode performing a disk and permission repair. No help after that.

I had previously fixed the Pro Video 2.0.1 update by installing them manually and my video apps now work. however Mac apps still crash Yosemite.

Running on Mac Pro (Late 2013), OS X Yosemite 10.10.3

Please help if you have similar problems with a solution.

Any help is very much appreciated in advance.

Mac Pro, OS X Yosemite (10.10.3)

OK. Today I finally had a crash again - after the system was running for over a week straight.

Today it was DEFINITELY due to a web site trying to start a Flash Player app. More specifically a streaming audio file using a flash player from a radio station web site that I have used regularly.

One new observation: Upon doing a hard reboot - my Mac Pro was extremely HOT. So hot it was almost burning to the touch. I felt this on accident as I put my had on top of the machine while pressing the power button. Being a Mac Pro (Late 2013) it had the open air duct in the top. I could feel super heated air coming out and when I put my fingers down inside the opening a bit it was almost too hot to touch.

Now it's been operating normally all day and not as hot.

I've uninstalled Flash completely for the time being.

I've failed twice in trying to upload this previously and the reason I try to do it is I notice the process is awdd while the symptoms are the same.  I will try a reduced version this time as perhaps the intro gives some clue.

Above is not relevant.  There are no logs such as above for any of the crashes today.  However, prior to the last two crashes, the console log is flooded with this:

    5/17/15 12:33:36.306 nsurlstoraged[285]: Error: stepSQLStatement:toCompletionWithRetry - stepping returned unhandled result=11, DB=/Users/alanfraser/Library/Caches/com.apple.Safari/Cache.db

I noticed prior to the crash before this one, this was mixed in the list.  There may have even been a hundred or so of the other but this one I only saw once:

    5/17/15 11:32:25.485 nsurlstoraged[285]: writeDBwithCachedResponse-ERROR with UPDATE for item: http://feedjit.com/dmgs/b1_D09100_5C5A5A.png[searchingforithaka.blogspot.com].  Error-code:11

Feedjit is a common add-on for Web sites.  I've run it for years so it's unusual Safari would choke on it now.

Common to every startup are Skype and Safari.  All peripherals have been taken offline and there has been no change to crash frequency.

At the crash point, the screen goes black and I have to disconnect power for twenty or thirty seconds before I can restart.  The symptom has been identical every time and there were many, many crashes in recent weeks, at least half a dozen today.
