You have multiple incompatible versions of Pages on your Mac.

Pages '08, or '09 documents contain the index.xml file, and Pages v5 documents do not. You have tried to open a Pages v5 document in an older version of Pages. Pages v5 documents are not directly backwards compatible with earlier Pages applications, and must be opened with Pages v5.

Pages '08 documents are not supported in Pages v5, and must first be exported to MS Word from Pages '08, or opened in Pages '09, and then saved as a Pages '09, or exported as MS Word documents.

If you open a Pages '09 document in Pages v5, and let the latter convert it to Pages v5 document format, you can only open it in Pages '09 again via export from Pages v5.
