I thought maybe because the bluetooth from your phone can't be accessed because you are on the phone it wouldn't allow you to see an incoming call on the watch, but then how is the watch able to give you a notification that you missed a call when you are still on the phone.

If your iPhone is locked, then all calls get prioritized to alert you first on the Watch, including additional calls. If you are taking the call on your iPhone though, iPhone gets the priority for call notifications and your watch will not alert you to the second call. The notification system for multiple devices is based on priority for what is currently unlocked or in use.
