I just want to create a backup from my iPhone but i don't want to have all the photos in this backup. Is it somehow possible to deselect the photos?

As far as I konw, you can't do that. You can only restore the lastest back up. This will include all the stuff you restored
If you are referring to reducing the storage required when creating an iCloud backup: Settings > iCloud > Storage > Manage Storage > select device in the Backups section > Backup Options will allow you to select items you don't want backed up. Note that if you have iCloud Photo Library turned on for the device, photos aren't included in a backup to iCloud.
