My iPhone no longer uses my phone number for iMessage or FaceTime

I'm not sure exactly when this happened but I suspect it may be related to one of the later iOS upgrades (I am now using iOS 8.3 on an iPhone 5). iMessage and FaceTime no longer work with my phone number (activation fails). The only way I can use them is with my email address. I have tried resetting my phone and turning off my Apple ID but to no avail.

Hi Jonathan Klein3,

Thanks for using Apple Support Communities. Based on what you stated, it sounds like Activation is failing with those two applications. I would recommend that you read these articles, they may be helpful in troubleshooting your issue.
