Sounds like you've done some great troubleshooting steps to resolve your issues with the camera on your iPhone. The following Knowledge Base aritcle provides some additional information for you:

You see a closed lens or black screen
If you see a closed lens or black screen when you open the Camera app, try these steps:
- Make sure that there’s nothing blocking the camera lens. If you’re using a case, try removing it.
- Force the app to close, then open the Camera app again.
- Restart your device, then open the Camera app again.
- If your device has a front and rear camera, try both cameras by tapping
(camera swap icon). If you see the closed lens or black screen on only one camera, take your device to an Apple Retail Store or Authorized Service Provider for more help.

Since this isn't happening to the rest of us and has continued for you after restoring to factory settings without using a backup, you clearly have a hardware problem that needs evaluation by Apple.

May 3, 2015 4:33 AM in response to Harkey013by Harkey013,Went to the Apple Store, the Guru said he'd seen this before and was a HW fault in the front camera. As the front camera and screen are combined it would need replaced at a cost of £99 as it was 27 days out of warrantee, I therefore took it back to the Phone shop I got it from who also said it was out of warrantee until i reminded him it was covered for 2 years under the sale of goods act. He still wasn't sure but filled out the repair form which confirmed what I said. Result, don't always take the sales assistants word for it, do your own research.
