My caller ID comes up private when I make calls or send emails. How do I unblock?

When I send emails or make calls from my iPhone the caller ID comes up as PRIVATE on the phone or computer I'm contacting. I don't want them to be private. I want my name or phone # to show. "Show Caller ID" is toggled green under phone settings. I'm stumped.
