How do i get rid of my Documents & Data on iPad and iPhone? everytime i plug it in to my Mac i would open iTunes to see if my documents and data are gone does it have something to do with iCloud

Yes, it has to do with iCloud.
Check Settings > iCloud > Storage > Manage Storage

You can synchronize documents and data with iCloud but the items taking up physical storage on the device are "other" file that are not movie, music, or photos. It could be iOS updates you haven't installed, iWork documents, or cached information. If it is becoming an issue you can restore the device in iTunes and set it up as a new device.
