My wife bought an Ipad one used from a co-worker who reset the device and now the puk and sim are locked what should I do?

It is probably the Activation Lock. The former owner must remove the device from his account. Otherwise it will be unusable without the Apple ID and Password.

Not likely, what is probably happening is that the sim (cellular network account) is locked. What they'll probably need to do is to buy a new sim card and subscribe to a new network plan.
