It sounds like your bluetooth connection is dropping between your AppleWatch and your iPhone and won't stay connected reliably. I would start by restarting both the phone:
iOS: Turning off and on (restarting) and resetting
And your AppleWatch, then test out the connection:
Restart or reset your Apple Watch
If that doesn't get the connection to be more consistent then I would next use the bluetooth troubleshooting in the following article on the iPhone:

This has been happening to me and resetting networking settings and restoring both my iPhone 6 and watch has not helped! I am trying to figure out if this is a software issue or a Bluetooth issue (and with which device)
