why did AppleTV remove YouTube from its menu? and why does CraveTV appear on the new menu
but without a tv service provider, I can't access CraveTV the way I can access Netflix?

What has happened is Google ended support for the older YouTube applications. The YouTube app on the 2nd generation Apple TV as well as the YouTube app that Apple preinstalled on older iOS devices will no longer work. Apple just went ahead and removed the Apple TV app to avoid frustration. The best workaround is to AirPlay a video from your iOS and OS X devices up to the Apple TV. My personal opinion, the older Apple TV app was terrible.
