Itunes Randomly Deleting Music

ios 8.3/ iphone 5

Randomly when I go to play a song in itunes, I select an available song, not just in cloud, and all of a sudden I see the total available count go down, my song selection skips to the next available, and the song I wanted, plus others are removed and only available via the cloud. My phone is not tethered, I have 10.7 gb available space. I do not understand why this is happening. Later when I check the song on my Macbook Pro it is still on the harddrive and available in itunes.

I get random holes where I have to redownload music and it is driving me nuts. Has happened a few times.

User error some how?
